How did it happen that, besides the main work at RUDN, you also became the scientific director of the anatomical course for cosmetologists in Prague?
The idea of creating separate anatomical courses for cosmetologists just arose while teaching at the Faculty of Advanced Training of Medical Workers at RUDN. I was asked about this by the doctors themselves, whom I taught the features of thread technologies. It so happened that my clinical experience and education combines the practice of a maxillofacial surgeon (residency in maxillofacial surgery and then 10-year work experience), and an anatomist (postgraduate study and an assistant at the department of topographic anatomy and operative surgery), and a dermatologist (internship and practical activities).
I understand very well how to explain this knowledge in practice. Five years ago, Natalia Markova, the head of Medical Aesthetics, and I organized such a training course for doctors in Prague.
Why do doctors need it? Is learning at the institute in anatomical laboratories, not enough?
All future doctors are studying anatomy at the 1 and 2 courses in the medical institute. But only a few students are aware of what exactly will they do in the future. Moreover, students do not study important individual questions of anatomy, for each specialization. Residency in dermatology with a course of cosmetology does not yet provide additional classes on facial anatomy in anatomical laboratories, only theoretical lecture hours. Meanwhile, the face is a very complex area, large vessels and nerves are located superficially. Accordingly, the likelihood of complications as a result of procedures. Why do anatomical courses take place in the Czech Republic?
Our courses have a 5-year history, and the idea was discussed earlier. Medical Esthetic, together with Karlov University, already had experience in conducting educational events for dermatologists, so we decided to start cooperation with them.
You conduct anatomical courses for cosmetologists in Prague six times a year. Is this so in demand?
The interest in cadaver courses among cosmetologists is enormous. Their relevance is associated primarily with the development of cosmetic technologies. The fact is that aesthetic medicine is rapidly developing, the work of cosmetologists are carried out in deeper layers. Practicing doctors clearly understand what knowledge they lack, and work on anatomical material fills these gaps.
The fact is that in Europe a dermatologist has the right to violations of the skin and can conduct small biopsies. In our country, a dermatologist has always been considered only a therapeutic specialty. Surgeons operated, and cosmetologists basically did masks and massages. But with the development of new technologies, with the advent of a wide range of various injection interventions, the improvement of pharmaceutical materials, the work of a cosmetologist has ceased to be limited to superficial skin. Beauticians now work with subcutaneous tissues, such as fat tissue and muscles. And many patients now prefer minimally invasive techniques that give a good result.
Plastic surgeons in this regard, also, drew attention to the minimally invasive technology?
Yes, today almost all plastic surgeons, along with conducting surgical interventions, are engaged in injection cosmetology. Of course, plastic surgery often gives a more effective and long-term result. You conduct anatomical courses for both Russian and foreign doctors. Do Russian and Western approaches differ in aesthetic medicine?
I teach doctors from Europe a lot and I can say that the approaches differ not only in aesthetics but also in general medicine too. For European doctors, standards are very important. For each pathology, they ask for a standard - a clear procedure for dealing with a problem. This is dictated by the requirements of insurance medicine. In many countries, aesthetic medicine services are covered by insurance. Therefore, the doctor must have evidence for the insurance company that he acted strictly according to the protocol corresponding to this or that problem. On the one hand, it regulates the approaches to the treatment of the patient, on the other - it makes doctors relaxed. In fact, he does not need to think why he acted this way if he followed the rules. In Russia, the patient himself pays for the services of aesthetic medicine. It is important for our doctors to achieve the most effective result because it does not report to the insurance company, instead reports directly to the patient.
Does this mean that patients benefit from the Russian approach?
In general, yes, because each case is individual. There is no single protocol. If we are talking about cosmetic procedures, it is impossible to clearly relate them by age. What interests listeners of anatomic courses most of all?
It is very important to explain for doctors how to increase the effectiveness of procedures, to carry out a combination of techniques more effectively. The course was conceived and created precisely as an anatomical substantiation of thread methods, their explanation, and training on anatomical material. In addition, we tell and show work with botulinum toxin and fillers, but one way or another, all this is considered in the context of combination with thread technologies. I remember the humorous phrase of one of our doctors after the course: “If earlier I was afraid to even look at the thread, now I can implant them with my eyes closed and in a dark room”. In the anatomical course, we teach how to feel the tissue tool. This is the main goal of the course. We work subcutaneously, so the eyes of a cosmetologist are his tools.
